1. Call to Order and Invocation:
Mr. Price called the meeting to order Mr. Wright gave the invocation.
A motion was made by Mr. Warner and seconded by Mr. Wright to add to agenda under Director of Nursing: Approval of Nursing Policies and Approval of PI report; under Executive Session: Strategic planning Re: Chest Pain Accreditation. Motion carried unanimously.
2. Approval of Minutes:
A motion to approve November 2, 2011 meeting minutes was made by Mr. Wright and seconded by Mr. Poulos. Motion carried unanimously.
3. Director of Nursing Report: April Winborne, Director of Nursing
A motion was made by Mr. Poulos and seconded by Mr. Wright to approve the CHF Protocol Orders as presented. Motion carried unanimously.
A motion was made by Mr. Poulos and seconded by Mr. Wright to approve the Routine Orders as presented. Motion carried unanimously.
4. Medical Staff: Dr. Roger Smith, Chief of Staff
A motion to approve the credentialing of Dr. John Armstrong, Dr John Maxwell, Dr Amir Fassihi, Dr John Lewis, Dr James Turner, Dr James Neal, Belinda Morgan, FNP, Lisa Faulkenberry, FNP, Janna Chauvin, FNP, Dr Rita Agarwall, Dr Scott Zentner, Dr Tom Wooldridge, Dr Tom Colvin.
5. Financials: Blake Kramer. Administrator
A motion was made by Mr. Poulos and seconded by Mr. Warner to approve the financials as presented. Motion carried unanimously.
A motion to approve the mileage reimbursement amount as recommended was made by Mr. Wright and seconded by Mr. Warner. Motion carried unanimously.
6. Personnel Update: Terri Spence, Personnel Services Officer
A motion was made by Mr. Wright and seconded by Mr. Poulos to approve the applicant statement of expectations as presented. Motion approved unanimously.
7. Operations Update: Mark Spence, Operations Officer
A motion was made by Mr. Wright and seconded by Mr. Poulos to approve the quote on portable ultrasound equipment as recommended by Mr. Spence. Motion carried unanimously.
8. Administrative Report: Blake Kramer. Administrator
A motion to approve the Urinalysis quote as presented by Mr. Kramer was made by Mr. Poulos and seconded by Mr. Warner. Motion carried unanimously.
A motion to approve Coagulation machine quote as presented by Mr. Kramer was made by Mr. Warner and seconded by Mr. Wright. Motion carried unanimously.
A motion to approve the Telemetry upgrade was made by Mr. Wright and seconded by Mr. Warner. Motion carried unanimously.
9. Executive Session:
A motion was made by Mr. Poulos and seconded by Mr. Wright to enter into Executive Session. Motion carried unanimously.
10. Open Session:
A motion was made by Mr. Poulos and seconded by Mr. Wright to enter into Open Session. Motion carried unanimously.
A motion was made by Mr. Poulos and seconded by Mr. Wright to approve the transfer agreement to Glenwood as discussed in Executive Session. Motion carried unanimously.
11. Public Comment:
No public comment heard
12. Adjourn:
A motion to adjourn was made by Mr. Poulos and seconded by Mr. Wright. Motion carried unanimously.

The Village of Gilbert (herein referred to as the “Owner”)
Sealed bids marked “Sealed Bids” - - Village of Gilbert Community Development Block Grant Project for Fiscal Year 2011 to be financed by the State of Louisiana CDBG Program will be received by the Owner for the construction of the project as follows:
LCDBG STREET IMPROVEMENTS
Proposals shall be addressed to the Village of Gilbert , and delivered here to the City Hall located at 7564 Gilbert St., Gilbert, LA 71336 , not later than 10:00 o’clock a.m., (local time) on February 7, 2012 . Proposals shall be designated as “Sealed Bid” - - LCDBG – Street Improvements , Louisiana Community Development Block Grant Project for Fiscal Year 2011 to be financed by the State of Louisiana CDBG program. All bids must be submitted on the proper bid form. The contractor shall display his contractor’s license number prominently on the outside of the envelope. Any bids received after the specified time and date will not be considered. The sealed bids will be publicly opened and read aloud at 10:00 o’clock a.m., on February 7, 2012 , in the City Hall located at 7564 Gilbert St. located in Gilbert, LA 71336. The Information for Bidders, Form of Bid Proposal, Form of Contract, Specifications, Forms of Bid Bond and other contract documents may be examined at the office of the Engineer , located at the 114 Venable Lane, Monroe, Louisiana 71203.
Copies may be obtained at the office of Denmon Engineering located at 114 Venable Lane, Monroe, Louisiana 71203 , upon deposit of $50.00 for each set. Please mail all requests and deposits for plans to the mailing address, P.O. Box 8460, Monroe, Louisiana 71211. Deposits on the first set of documents furnished to bona fide prime bidders will be fully refunded upon return of the bid documents, if returned in good condition and not later than 10 days after receipt of bids. On other sets of documents furnished to bidders, the deposit less the actual cost of reproduction ($25.00 ) will be refunded upon return of the bid documents, if returned in good condition, and no later than 10 days after receipt of bids.
The Owner reserves the right to reject any or all bids. The Owner shall not waiver the signing of: 1) the bid; 2) certificate of corporate principal and surety; 3) any addenda issued; and 4) bid bond which always must accompany the bid in the correct amount.
A corporate resolution authorizing a representative of the corporation to sign the bid must accompany the bid, if the bidder is a corporation. Such activities will be in accordance with Title 38 of the Louisiana Revised Statues.
Each bidder must deposit with his/her bid, security in the amount, form and subject to the conditions provided in the Information for Bidders. Sureties used for obtaining bonds must appear as acceptable on the Department of Treasury Circular 570.
Attention of Bidders is particularly called to the requirements as to conditions of employment to be observed and minimum wage rates to be paid under the Contract, Section 3, Segregated Facilities, Section 109, Executive Order 11246, and all applicable laws and regulations of the Federal government and State of Louisiana and bonding and insurance requirements.
No Bidders may withdraw his/her bid within Sixty (60) days after the actual date of the opening thereof.
The Contractor shall begin mobilization and procurement of materials within ten (10) working days of the receipt of the Notice to Proceed.
The successful bidder will be required to execute the Owner’s Standard Form of Agreement/Contract for construction together with the Performance and Payment Bond within ten (10) working days after normal award of contract.
THE SUCCESSFUL BIDDER MUST SUBMIT EXECUTED COPIES OF CERTIFICATION REGARDING EQUAL EMPLOYMENT OPPORTUNITY, SECTION 3 AND SEGREGATED FACILITIES AND SECTION 3 PLAN WITH TABLES A & B.
MINORITY OWNED FIRMS AND SMALL BUSINESSES ARE ENCOURAGED TO BID ON THIS PROJECT.
